The ratchet mechanisms for wheel tie-downs are difficult to use at best. In this case, the right (starboard) ratchet was worn or bent such that, when straightening the device and lifting the release triggers, the cam that holds the mechanism free for release would fail to stay in place and cause the release to repeatedly fail. These ratchets require more frequent inspection/maintenance/replacement.
