I clicked yes for cleanliness, but I am not sure about well maintained as I received a text from Kevin all excited about me returning the rental for maintenance, so I don't know about it's well maintained status. I do know the headlights are out of alignment because I had the unfortunate experience of driving in the rain at night. The driver's side was pointed to the tree tops and the passenger side was pointed almost straight down. Other than that, and the transmission not moving into the cruise setting, so it stayed in the burn as much gas as possible range, this was a decent experience. The secretary was the best, very friendly, courteous, and helpful. Don't let one truck sway you from renting here, it could have happened to any location and I was being asked to return it on time for maintenance.
